In order to file a complaint, please, fill in the form below and attach all documents relevant to your case.
In order to examine your individual complaint on its merits, you should provide your name, permanent address, postal address and your e-mail address, if any. In the absence of this set of information, substantive examination of your complaint may be rejected. 
In case of police complaints, it is not possible to assess the merits of the complaint without receiving the data indicated (name, permanent address and postal address).
Your complaint and documents submitted to your case will be registered by the Office of the Commissioner for Fundamental Rights.
